This form will take about 10 minutes to complete. Please ensure you have the following documents before you begin filling in the form:

1. For all applications: Leader's recommendation & Member's recommendation

2. For Church Membership application only: Water Baptism certificate

3. For Church Membership Transfer application only: Water Baptism certificate and Letter of Transfer from your previous church
